Network Participation & Hospital Affiliations

As of April 1, 2015 the New Health Insurance Law requires physician to notify our patients of our hospital affiliations and insurance networks affiliations.

There may be occurrences when your neurological symptoms may require us to refer you to the emergency room. We are affiliated with South Nassau Communities Hospital; however you may be directed elsewhere. Though emergency care is usually covered we cannot determine whether the hospital is in your insurance network, it is your responsibility to contact the hospital directly or your insurance carrier with respect to network participation.

We often refer our patient to other specialists for a consultation and other facilities for diagnostic testing. You are required to contact their office or your insurance carrier for network participation before scheduling an appointment.

PLEASE NOTE FOR UNITED HEALTHCARE MEDICARE COMPLETE REGIONAL PPO PLAN I, II AND III. WE ARE CONSIDERED OUT OF NETWORK PROVIDERS AND YOU WILL BE RESPONSIBLE FOR CO-PAYMENT, CO-INSURANCE, AND COST SHARING.
